Abstract

Diabetic renal failure, also known as diabetic nephropathy, is a serious and progressive complication of diabetes mellitus that significantly impairs kidney function and leads to end-stage renal disease (ESRD). Among individuals in the productive age group (typically 20–60 years), this condition presents not only a major health burden but also has profound socio-economic and psychological implications. This thesis investigates the relationship between diabetic renal failure and the quality of life (QoL) in individuals of productive age, aiming to provide a comprehensive understanding of how renal complications arising from diabetes affect physical, emotional, social, and occupational functioning. The study employs a quantitative analysis through standardized QoL assessment tools such as the WHOQOL-BREF Urdu version. A cross-sectional sample of patients diagnosed with diabetic renal failure in urban and semi-urban healthcare settings was selected. Variables such as , diabetes, renal failure, age, employment status, gender differences, Socioeconomic background, education and social support were analyzed in relation to their impact on different QoL domains. Data analysis involved Pearson Product-Moment Correlation to examine the strength and direction of the linear relationship between illness duration and each QoL domain (physical, psychological, social), Independent Samples t-test To compare the mean QoL scores between male and female participants and determine whether the difference is statistically significant, Multiple Linear Regression & ANOVA (from regression model) To assess the predictive effects of income and residence (as categorical variables) on QoL scores, and to test overall model significance. Independent Samples t-test to reveal a statistically significant difference between education levels and DDRF scores. Preliminary findings reveal a substantial decline in QoL among diabetic renal failure patients in the productive age group, particularly in physical functioning, emotional well-being, and role limitations due to health. Dialysis dependency, financial strain, job loss or absenteeism, and psychological distress emerged as significant factors contributing to reduce QOL. Furthermore, gender differences, access to healthcare, and education level were observed as moderating influences. This research underscores the urgent need for integrated disease management strategies that not only focus on glycemic and renal health but also address the broader psychosocial needs of patients in their working years. The findings have implications for public health policy, particularly in the design of targeted interventions, patient education, and support programs to mitigate the impact of diabetic renal failure on the most economically active segment of the population.
