Abstract

Early childhood adversity is a critical determinant of socio-emotional development, particularly in low- and middle-income settings where multiple risk factors often coexist. This study presents a longitudinal examination of early childhood adversity and socio-emotional development among young children in Sindh, Pakistan. Using a two-wave longitudinal design, data were collected from 200 children aged 3–6 years and their primary caregivers in the districts of Hyderabad and Tando Allahyar over a 12-month period. Early childhood adversity was assessed across household socio-economic conditions, caregiver-related factors, environmental and community risks, and psychosocial neglect. Socio-emotional development was measured using the Strengths and Difficulties Questionnaire (SDQ). Data were analyzed in R using descriptive statistics and linear mixed-effects models to examine developmental trajectories and associations over time. Results indicated that early childhood adversity was multidimensional and persistent, with over half of the children experiencing adversities in two or more domains. Although modest improvements in socio-emotional outcomes were observed over time, a substantial proportion of children continued to exhibit moderate to high levels of difficulties. Higher cumulative adversity was significantly associated with increased total difficulties (β = 0.75, p < 0.001), emotional symptoms, conduct problems, hyperactivity/inattention, and peer relationship problems, as well as reduced prosocial behavior. These associations remained significant after controlling for key demographic and household factors. The findings underscore the long-lasting influence of early adversity on socio-emotional development and highlight the need for multilevel interventions targeting caregivers, households, and community environments. This study contributes longitudinal evidence from a low-resource context and emphasizes the importance of early preventive strategies to promote positive developmental outcomes.
